Eligibility Specialist Program
For families who include a child with a developmental disability and are seeking service coordination through Missouri’s Department of Mental Health – Regional Offices, or just wanting certification of a developmental disability through MO-DMH, F.A.C.T. can assist through this process! F.A.C.T.’s Eligibility Specialist assists families with the application and approval process by helping gather the needed documentation to determine eligibility and supporting the family through the various steps of the process.

Who can F.A.C.T. help?
You can qualify for this free program if your child meets eligibility requirements. Eligibility is based on funder requirements and diagnoses, not parental or family income. Any parent, or guardian, or agency can make a referral using our referral form. Fill out the basic information and we will contact you.
